Outpatient Coder II

Position Purpose:
Incumbents in this position shall comply with all applicable laws and rules including the Dignity Health Standards of Conduct and Dignity Health policies and procedures that apply to his/her job responsibilities, seek guidance when in doubt, promptly report any potential or suspected violation of Dignity Health Standards of Conduct, Dignity Health policy or procedure or applicable laws/regulations and when requested, assist Dignity Health personnel in investigating all allegations of violation. When called upon, the incumbent shall assist in risk evaluation and work collaboratively with facility/entity leaders to implement, monitor and mitigate risk corrective plans outlined by the Compliance program.

The Outpatient Coder II accurately determines ICD-10-CM diagnosis, and ICD-10-PCS, CPT and HCPCS procedure codes for all outpatient medical records that could include ED, Ancillary, Labs, Radiology, Catheterization Labs, low level Interventional Radiology, Urgent Care, Recurring Accounts, Observations, Rehab, Critical Care, Psych, and Trauma and, on occasion, charging for services and supplies. The Outpatient Coder II understands the Medicare Ambulatory Payment Classification (APC) codes. Some experience with Interventional Radiology is also preferred

Nature and Scope:
This position reports to the Coding Supervisor.

Primary Responsibilities: developing and maintaining good communications and working relationships with the medical staff, administrative staff, outside agencies and with other departments.

K ey Responsibilities:
1. Identifies and provides appropriate and accurate assignment of ICD-10-CM and CPT diagnosis and procedure codes for outpatient facility services provided in the acute hospital setting, as well as additional data elements identified during the chart review process

2. Follow official coding guidelines, including the Coding Clinic, CPT Assistant, and HCPCS

3. Enter all appropriate information in accordance with the policies and procedures of the medical facility.

4. Working knowledge of medical terminology, and anatomy and physiology

5. Familiarity and usage of multiple computer systems used in Coding (EHR, Encoder, Abstracting and Billing)

6. Maintain compliance with both external regulatory and accreditation requirements, and State and Federal Regulations

7. Maintains up-to-date coding knowledge by reviewing materials disseminated and/or recommended by Corporate Compliance and coding directors, managers, and supervisors

8. Queries clinical staff to achieve accuracy in coding

9. Incorporates quality assurance information and maintains quality and productivity standards as outlined by the corporation

10. Participates in coding department meetings and educational events

11. Maintains professional certification, as applicable

12. Contributes to the Quality improvement activities of the department and the organization, including participating in internal department and corporate audits and reviewing and correction of daily generated error reports

13. Communicates effectively and professionally with remote facility site managers, direct management, and fellow employees

14. Can independently organize and prioritize work to ensure records are coded in timeframes that will assure compliance with facility and regulatory standards

15. Access to High Speed Internet required

Qualifications:
Academic:
1. High School Diploma or GED

2. Health Information Technology or certified coding program degree

3. Will consider the following certifications:
a. Certified Coding Specialist-CCS

b. Registered Health Information Technician or Admin-RHIT or RHIA

c. Certified Professional Coder-CPC

Experience:
1. At least 2 years’ experience working in an acute hospital setting coding outpatient procedures.

Knowledge, Skills and Abilities:
1. Appropriate training to perform the necessary essential functions

2. ICD-10 Training

3. Demonstrated competence with personal computers, networks, and Microsoft Office.

4. Experience with various coding abstracting and coding systems (MS4 Mira, Meditech, Clintrac, Optum CAC, HBO Star) and electronic medical record (Cerner Powerchart)
