HS TECHNICIAN – HIV/STD COMMUNITY PROGRAM

The

Education and Outreach Team
supports the
HIV and Sexually Transmitted Diseases (HIV/STD) Community Program
within
Wake County Human Services Department – Public Health Division
. The
Human Services Tech (Outreach Navigator)
provides street and community outreach to reduce the risk of Human Immune-deficiency Virus (HIV) /Sexually Transmitted Diseases (STDs) by engaging high-risk populations into prevention programs and helping them to navigate essential services to address some of the social determinants of health that impact their behavior. The Outreach Navigator will focus on priority populations as defined by Center of Disease Control (CDC) (young black and Latinos Gay and bisexual men who have sex with men (MSM) and transgender women who have sex with men (TWM). This position will require use of personal vehicle for travel.

Essential duties include:

• Provide educational and resources information.

• Engage potential clients into HIV prevention programs and care.

• Provide Pre-Exposure Prophylaxis (PrEP) and Post-exposure Prophylasis (PEP) education.

• Identify new community partners for collaboration with the HIV/STD program.

• Assist Communicable Disease and clinic staff to locate MSM, commercial sex workers, homeless, HIV-positive persons and Latino men who may need treatment or be at increased risk for HIV and other STDs.

• Provide educational and resources information.

•Ability to convey HIV/STD and health information clearly and concisely to individuals and groups to ensure they understand the information and the message
