JOB DESCRIPTION:

  • Tasked with leading patient outreach activities for patients attributed to the Emory Healthcare Network (EHN).
  • This includes working with health plans to identify all patients assigned to EHN.
  • Outreach takes many forms, including telephonic as well as via coordination through the patient’s primary care physician (PCP).
  • The position is tasked with getting as many of our assigned patients as possible to present to their attributed PCP yearly for comprehensive annual visits.
  • From there patients may be further enrolled in disease management or case management services depending on need.
  • The navigator also must engage in critical thinking to select the patients both most likely to come to the clinic as well as most likely to benefit from our services.
  • All outreach activities must be meticulously tracked in an excel document.
  • The navigator then independently audits and assesses the data in quality improvement activities, to determine which methods of outreach result in optimal patient engagement.
  • They then self-implement process changes to improve outcomes.
  • The navigator position is also ultimately responsible for all reporting of quality metrics to health plans contracted with the EHN.
  • This involves working closely with multiple plans to identify Healthcare Effectiveness Data and Information Set ( HEDIS ) gaps (as defined by CMS), indicating either a gap in care delivered to the patient or a gap in reporting to the plans.
  • The navigator performs chart audits, and if the care has been provided, they close the reporting gap through mechanisms of the plan’s choosing.
  • If it is a gap in care delivery, they outreach to the patients, or work with the PCP, to bring them to the clinic to enroll them in appropriate care.
  • The navigator also works with other members of the interdisciplinary care team at the EHN to enlist their assistance in various patient engagement strategies, requiring some degree of leadership and teamwork skills.
  • Maintains a clean and healthy environment in compliance with Universal Precaution guidelines and safety standards.
  • All duties and patient interaction performed in accordance with Emory Healthcare patient and family centered care standards.
  • May participate in research projects impacting patient care areas.
  • As part of a patient focused service, promotes a supportive and healing environment to enhance customer and staff satisfaction.
  • Performs other related responsibilities as required.
  • MINIMUM QUALIFICATIONS:

  • Bachelor’s degree highly preferred.
  • Two years of similar experience highly preferred.
  • No medical experience required. Outstanding interpersonal skills, both telephonic and in person, required.
  • Basic computer skills, including Microsoft Excel, required. Must be a true team player.
  • PHYSICAL REQUIREMENTS (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs, 34-66% of the workday (frequently); 01-10 lbs, 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks.

Environmental exposures include, but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock, Floor Surfaces, Hot/Cold Temperatures, Indoor/Outdoor conditions, Latex, Lighting, Patient care/handling injuries, Radiation, Shift work, Travel may be required. Use of personal protective equipment, including respirators, environmental conditions may vary depending on assigned work area and work tasks.
