Patient Care Coordinator – 4th Floor Pediatrics and Family Medicine

The Patient Care Coordinator collaborates with the multidisciplinary team to begin discharge planning; communicates with referring physicians, facilities and agencies regarding patient care; performs daily rounds; participates in legal/risk occurrence tracking and monitoring patient outcome measurements; functions as a resource; oversees and organizes the flow of workload; coordinates post-discharge calls; demonstrates an understanding and assists the Manager in keeping within the fiscal/budget constraints, and mentors staff and assists in providing professional growth and education opportunities. This includes caring for patients in the following age groups: infant, children, adolescent, adult, and geriatric.

Primary Job Duties:

– Collaborates with the multidisciplinary team to begin discharge planning.

– Communicates with referring physicians, facilities and agencies regarding patient care.

– Performs daily rounds on all patients assuring quality customer service.

– Participates in legal/risk occurrence tracking and monitoring patient outcome measurements, conducts follow-up and assures appropriate action.

– Functions as a resource to staff, physicians, and patients.

– Oversees and organizes the flow of workload according to the staffing matrix.

– Coordinates post-discharge calls with appropriate follow-up.

– Demonstrates an understanding and assists the Manager in keeping within the fiscal/budget constraints.

– Mentors staff and assists in providing professional growth and educational opportunities to staff and students.

KNOWLEDGE & SKILLS REQUIRED:

Required:

• Registered Nurse.

• Licensed by the North Dakota Board of Nursing or licensed by a member of the “Compact State Agreement”.

• Must obtain Minnesota license from Minnesota Board of Nursing within eight (8) weeks from their first day of work.

• Telephone calls from patients calling from Minnesota cannot be taken from any nurse unless they have obtained their Minnesota license.

• A minimum of three (3) years of clinical experience for Obstetrics in maternal child care, including Labor and Delivery and three (3) years in Neonatal Intensive Care Unit (NICU) and one (1) year pediatrics experience for Peds.

• Demonstrates ability to effectively communicate both verbally and in written format.

• Successful completion of the functional assessment.

Preferred:

• Bachelor’s Degree in Nursing.
