Mobile Associate Counselor

This is a traveling counselor position with the responsibility of providing in-home counseling services to general mental health clients throughout the valley. Responsible for treatment planning, individual, family and group counseling, and coordinating with other involved professionals to secure supportive services. This position keeps quality clinical records, charts monthly progress and oversees implementation of treatment and plans. Monitors and follows up on client needs, does re-assessments as required or as needed. Builds rapport and credibility simultaneously with our diverse member population. Relates to variety of individuals with warmth, empathy, acceptance and humor.

ESSENTIAL DUTIES & RESPONSIBILITIES:

Develop and implement successful intervention techniques including identifying the treatment method, establishing treatment plans that identify problems, goals and measurable objectives that are achievable within a reasonable time frame, interventions/strategies that address the assessment issues, and maintains timely progress notes that reflect movement towards goals/objectives.

· Conduct educational activities to promote client understanding of treatment issues including medication, diagnosis, parenting skills.

· Complete bio-psychosocial assessments, treatment plans and other clinical reports under the supervision of an independently licensed professional.

· Provide individual psycho-education, and crisis intervention and management services under the supervision of an independently licensed professional.

· Coordinate with a multidisciplinary team regarding patient care. Maintain patient confidentiality in the management of information and in accordance with HIPAA rules. Participate in mental health staff and department meetings. Adhere and model the code of ethics associated with license or certification. Meet regulatory, licensure, health assessment and CPR requirements. Other duties as assigned.

MINIMUM QUALIFICATIONS:

· Bachelor’s Degree in Sociology, Social Work, Psychology, Counseling or related field of study from an accredited college or university

· Experience working with mental health/behavior health crisis work

OTHER REQUIREMENTS:

· Must have or be able to obtain a DPS fingerprint clearance card (within 90 days of employment)

· Must possess a valid Arizona driver’s license and be insurable under the agency’s automobile policy.

· Must be able to work independently.

· Must be organized and proficient at creating own schedule as well as have a good understanding of the Phoenix-Metro area

· Must be proficient in MS Office and able to use basic office equipment and have daily access to a computer and Internet connection for documentation of services provided.

· The ability to work flexible hours based on the needs of the clients and families served, which may include evening or weekend hours
