Community Outreach Coordinator

Hennepin Health is seeking a Community Outreach Coordinator to lead our organization’s community engagement initiatives, which includes building relationships with key community organizations, potential members and other County departments. This is a new position that formalizes an emerging set of activities that link our health plan services and innovative care delivery model with the many stakeholders in the community we depend on for our success. This person will represent Hennepin Health within the community, as well as develop and execute a strategy to maximize outreach presence within the Hennepin County region where we operate. The position will be part of Hennepin Health’s Marketing and Communications Department, but will require the person to work closely with all departments, as well as with Hennepin County’s Human Services and Public Health Department (HSPHD), to ensure that community voices are considered in all health plan operations.

Hennepin Health, Hennepin County’s health plan, provides health care for some of the most socially and medically vulnerable citizens among us. Hennepin Health’s innovative practices have gained national attention and are considered a model for health reform.

The primary duties and responsibilities of this position include:

Develop and implement a community engagement and outreach strategy for Hennepin Health

Create and maintain strong relationships with community organizations and stakeholders, as well as with Hennepin County departments serving public health care program enrollees

Represent Hennepin Health within the community (e.g., at events, in homeless shelters, etc.)

Collect and document feedback that can then be shared with Hennepin Health leadership and staff as appropriate

Ensure that member services, provider relations, marketing/communications and other organizational teams are aligned with community engagement efforts

Serve as team lead for any additional outreach staff (e.g., organizing scheduling, logistics, materials needed, transportation, etc.)

Track and report metrics to quantify the impact of community outreach activities

Educate the public about Hennepin Health’s products, as well as its broader health reform vision and goals Best Qualified Candidates will have:

One of the following:
Bachelor’s degree or higher in a health, business, liberal arts or an approved related field. Note: Related experience performing community outreach or health education activities may be substituted for education on a year-for-year basis

Completion of the Career Connections Health Advocate Pathway Training Program may be substituted for one year of the required education

Ability to:
Work flexible hours, including occasional evenings and weekends as needed

Travel within the Twin Cites metro area to attend events.

Work effectively with a diverse client and customer population

Bilingual capabilities in Spanish, Somali and/or Hmong is preferred, but not required

Extensive knowledge of Minnesota Public Health Care Programs

Broad knowledge of the health system, health plan operations and county programs

Experience:
Performing customer service and/or community outreach for a health plan

Fostering existing relationships with key community partners within Hennepin County

A passion and strong commitment for working with the community, and a desire to improve the health care delivery system

Confidence in working with professionals of varying personal and career experiences

Strong self-direction, work ethic and intellectual curiosity Invitations to interview will be based upon an assessment of education and experience. Final candidates will be required to complete a background check post interview.
