Job Summary

Job Title: Community Based Rehabilitation Provider Objective : To provide community based rehabilitation services to participants who are recovering from a severe and persistent mental illness that promotes recovery, assists the member to integrate with his/her community, and provides services aimed at helping the member improve his/her quality of life.

Job Description:

A Community Based Rehabilitation Provider helps participants develop skills needed to increase their capacity to thrive in their home, employment, school or social environments. Services target skills for adults and youth that have been lost due to the symptoms of a behavioral health condition. Services vary in intensity, frequency, and duration in order to support participants in managing functional difficulties, or to otherwise realize recovery goals. A good rehabilitation provider should have excellent communication skills, problem solving skills, compassion, time management and organizational skills.

Minimum Qualifications:

Bachelor’s degree in Psychology, Social Work, Sociology, Family/Consumer Sciences, Child Development, Substance Abuse, mental health, human services, Education, Criminology, Counseling, and Therapy Fields. Job Description: Case Management consists of services that help consumers gain access to needed medical, social, educational, and other services. This includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services. Case Management includes: (1) referral and related activities to help the participant obtain needed services; (2) monitoring and follow-up; and (3) evaluation. Case Management does not include direct delivery of medical, clinical, or other direct services. Case Management is on behalf of the consumer, not to the consumer, and is management of the case, not the consumer. Qualifications:
