In the Care Advocate I role, you’ll provide telephonic, e-mail and/or community forums to outreach to members in an integrated care management model to reduce modifiable risks factors, encourage program engagement and participation through education, lifestyle management, and referrals to other internal services and resources.

The end result is to achieve personal health outcomes by providing support, encouragement, and education to improve well-being and prevent/minimize disease.

Responsibilities & Requirements:

• Care Advocate I would have the following certifications/degrees: Nursing Assistant (CNA) or a bachelor’s degree with emphasis on Physical Therapy, Health Care or Health Management and a minimum of one years’ experience in an office or clinic setting
  
• Knowledge of specific disease and lifestyle related topics such as smoking cessation, weight management, nutrition, pre-post-natal care, stress reduction and chronic conditions
  
• Strong communication skills with the ability to effectively interact with a wide variety of individuals
  
• Track record of customer service excellence
  
• Organization skills and proven ability to effectively handle multiple tasks and prioritize workload
  
• Knowledge of technology and applications such as Excel, Word and PowerPoint
  
• Understanding of care management principles
  
• Ability to flex hours to staff early evening calls
