Summary of Job:
Under general supervision supports individuals with hope, encouragement, support, while encouraging self-determination, developing physical wellness/fitness plans, and supporting meal planning as a means to increase physical wellness of citizens. Acts as an advocate for the needs and rights of persons served as they endeavor to increase their physical wellness.


Essential Job Functions

  • Acts as an advocate for the needs and rights of persons served as they endeavor to increase their physical wellness.
  • Assist citizen’s in developing their support system and wellness plan that meets their physical wellness goals, hopes and needs.
  • Assist citizen’s in developing a supporting meal plan as a means to increase physical wellness.
  • Coordinates with the citizen and their supporters to identify activities that will assist the individual in the physical wellness solutions they are seeking.
  • Assist individuals to engage in a whole person healthcare while supporting their connection to a Primary Care Physician whenever possible.
  • Support, model and teach physical wellness.
  • Model personal responsibility for physical wellness, self-advocacy, and hopefulness via telling of their recovery story and fitness journey, respectfully meeting needs, and being the example of physical health recovery.
  • Assist citizen’s in updating the Wellness Plan every 30 days and as the person accomplishes their goals and identifies new physical wellness solutions they are seeking
  • Provides supportive services and engages in related activities such as, supporting the person to access Wellness City recreation activities to assist in the development of a realistic healthy eating plan.
  • Maintains regular and consistent attendance and punctuality.
  • Driving of members to and from community events in personal vehicles, or utilization of the fleet van.

Documentation

  • Assists the individual in preparation of and recording of the Personal Wellness Plan and the documentation of the progress of that plan and any barriers to the person achieving the solutions they are seeking using Recovery Coaching
  • Ensures confidentiality of verbal and written individual information.
  • Documents relevant and required information and services provided in the identified service record (electronic or otherwise) assuring continuity of care as a tool for communication with other team members.
  • Maintain and observe all HIPAA regulations that apply.
  • Provide timely, quality and accurate documentation in accordance with program standards.

Safety/ Physical Environment

  • Participates as a team member to provide a safe recovery environment. Utilizes infection control and other safety standards. Contributes to the cleanliness of the work place.
  • Utilizes effective positive engagement and conflict resolution skills.
  • Fosters personal and environmental wellness by taking care of self and maintaining environmental wellness by reporting infection and on the job injuries.
  • Assesses emergency situations, notifies supervisor of actual or potential challenges, exercises judgment in stressful conditions and responds calmly and quickly. Displays teamwork during emergencies.
  • Influence and improve operations which are not under direct control.
  • Use problem solving methods and critical thinking to continuously improve operations.

Job Specifications


Education & Experience

  • Bachelor’s degree in physical education and at least one (1) year of full-time behavioral health work experience or;
  • High school diploma or GED and a certificate in fitness with full-time behavioral health work experience totaling at least one (1) year or;
  • High school diploma or GED and at least two (2) years of employment under supervision at a gym or fitness center, that included development of healthy eating plans in addition to behavioral health work experience totaling (2) two years or;
  • High school diploma or GED plus one year of experience and/or education in the development and facilitation of physical fitness classes, which includes healthy eating choices, exercise and fitness classes, and classes about increasing physical wellness plus one year full time behavioral health work experience.
  • Peer Employment Training Certificate preferred in addition to one of the above qualifications.

License & Certifications

  • Valid Arizona Driver’s license with clean driving record for the past 39 months as determined by the State Department of Motor Vehicle.
  • At least 25 years of age.
  • Crisis Prevention Management Training.
  • Food Service Worker’s permit may be required depending on worksite.
  • Certified CPR and First Aid
  • Facilitation Training certificate within 6 months of hire
  • Recovery Coaching certificate within 6 months of hire
    • Certification as a Fitness Educator/Fitness Trainer or similar from a school or program approved by Recovery Innovations within 6 months of hire. After 6 months, maintaining employment would be contingent upon completing the agreed-upon certification and maintaining it.
