    Job Title: Receptionist

    Job Purpose:

    This position is the initial point of contact of LIFE to the public, and manages a hub of activity. Answers main phone lines and receives all visitors when they arrive at the center, making them welcome ensuring they sign in and out. Determines the purpose of call/visit and sees that calls are routed to the appropriate staff person and visitors are met for escort into the center. Promotes the safety and security of participants and staff. Performs administrative duties for LIFE staff as assigned.

    Major Duties and Responsibilities:

    1. Promotes the LIFE program by offering responsive customer service at all times. Answers the main program telephone number, screening intake and all other calls, and serves as initial contact between the public and LIFE, and between staff , participants, and caregivers.

    2. Keeping track of all participants as they enter and exit the building. Ensure all name tags for participants are given to participants upon arrival and collected at the end of the day, sanitized and stored appropriately.

    3. Transfer phone to Health Center at the close of business daily.

    4. When new assessments are scheduled by the enrollment department, create a schedule for IDT members to see potential participant. Ensure each discipline has time to evaluate and that all IDT members see the potential new enrollee.

    5. Triages calls appropriately, interrupting meetings/activity as necessary to route important/urgent calls to clinical staff or other IDT members. Communicates with Health Center to locate medical staff for immediate response/intervention when needed.

    6. Uses intercom and Radio appropriately for communication within the building.

    7. Monitors security screen and the front door. Greets applicants, participants, family members and other visitors arriving at the LIFE Center. Screens all visitors and directs them to sign the visitor’s log, invites them to have a seat, and announces visitors to staff.

    8. Monitors the call bell enunciator panel for lights that do not go off within established timeframes. Responds and contacts internal first responders if alarm activates in any of the participant restrooms.

    9. Re-directs participants who are agitated or wandering in the lobby of the center. Prevents elopement, assuring that participants do not wander towards the front of the building or out of the main participant entrance. Notifies appropriate staff to assist with participants that may be trying to elope.

    10. Provides administrative support to Day Center staff and interdisciplinary team.

    11. Gather demographic information from applicants and participants as needed and documents in Truchart. Also uses TruChart to track daily attendance and services completed.

    12. Assists Transportation Manager/Dispatcher by keeping track of transportation needs and schedules for participants. Relays information between drivers, participants and LIFE team members.

    13. Completes Participant Absentee List by 10:00 a.m. and log attendance in TruChart.

    14. Records reason for absence and submit to Health Center for follow up.

    15. Compiles and collates weekly census for distribution to LIFE team. Copies and distributes all revisions to monthly participant attendance sheet.

    16. Perform any other related duties as needed.
