We are looking for an enthusiastic and highly motivated individual to join our multidisciplinary hospital palliative care team. The post is suited to someone with advanced palliative care skills, educated to degree level or working towards a degree, with a recognised teaching qualification and proven advanced communication skills.

The post holder will be required to:
Independently manage a patient caseload, advising medical, nursing and AHP staff on all aspects of palliative care.

Liaise with secondary and primary care agencies.

Teach on courses within the Trust, medical school and the Faculty of Health.

Support ward based staff and assist in development of generalist palliative skills across the Trust.

Contribute to the development of excellence in end of life care in liaison with members of the multidisciplinary team.

Contribute to the development of specialist palliative care within the Trust through participation in the ongoing development and delivery of the rapid discharge from hospital service. This will involve contributing towards delivery of education and training programmes in Advance Care Planning and appropriate use of theCo-ordinate My Carerecord.

Work independently on the weekend rota.

The team operates 9.00 – 5.00 Monday to Sunday.
