JOB SUMMARY

As an active member of the Social Work department, this position will oversee the development and review/revision of assigned treatment plans, and provide case management duties for the patients of the Arizona State Hospital

Responsibilities include:
– Coordinating of development, review, and revision of treatment plans in collaboration with the multidisciplinary treatment team

– Ensuring compliance with all licensing and regulatory body requirements for treatment plans

– Providing notifications to outside agencies

– Assisting in case management for coordination of patient care, admissions, and discharges to and from the hospital

– Providing department support according to level of education and credentials

Requires Knowledge Of:
– Understanding of mental illness and basic understanding of current theories of mental illness and treatment of persons with mental illness

– Knowledge of Performance Management (PM), Continuous Quality Improvement (CQI) and Lean methodologies.

Requires Skills In:
– Excellent verbal and written communication

– Diplomatic and professional demeanor

– Strong skill with computer programs in Microsoft office suite, and electronic medical records

Requires Ability to:
– Ability to work with a diverse patient population

Preferred Qualifications:
− Previous experience in case management and/or work experience in an inpatient psychiatric facility

– Preference will be given to candidates with prior Public Health experience

Employment Requirements

Candidate must have a Bachelor’s Degree in Psychology, Social Work, or related field.. A State Hospital employee must be able to obtain a valid Level One Fingerprint Clearance Card pursuant to A.R.S. § 41-1758.07.
