Community Health Manager (South Dakota)

About The Team: The Community Health Improvement team partners primarily with South Dakota community leaders, industry leaders, and citizens to strategically identify and implement well-being improvement programming and interventions that help move communities toward better health and well-being. The team works with communities and organizations to identify built environment and behavioral economic based interventions that can be implemented to make healthier choices easier, resulting in a long-term impact on the entire community.
About The Role: As Community Health Manager, you

’ll be responsible for managing the successful execution of community wide well-being improvement programs for multiple communities across South Dakota. You’ll partner with communities to identify built environment and policy opportunities that will transform the health of citizens, engage with community leaders, and consult on the implementation of identified opportunities. Additionally, you’ll leverage organizations and volunteers from the community to ensure local initiatives (consisting of multiple programs/projects) are being executed successfully as measured by engagement of citizens and organizations, project process and outcome measures, and other locally-identified metrics.

Minimum Qualifications Required (all must be met to be considered):

  • Bachelor’s Degree or direct and applicable work experience.
  • 4+ years of related experience:
    • Community/public program development. Experience partnering with key stakeholders (e.g., local government, business leaders, chambers, associations) and working with groups of volunteers to achieve goals. Ability to navigate government approval or public processes.
    • Strong group facilitation skills.
  • Strong consultation skills with the ability to influence and solicit cooperation from stakeholders. Must be able to understand the needs of others, ask meaningful questions, and distill information into actionable solutions.
  • Outstanding interpersonal skills with the ability to cultivate and manage key relationships. Professional, polished, and diplomatic in approach.
  • Strong verbal and written communication skills with the ability to articulate accurate information to stakeholders, both internally and externally, including key decision makers and diverse audiences.
  • Proactive, self-started able to work independently. Adaptable and flexible with changing priorities.
  • Strategic approach to work. Ability to support multiple communities with achieving short and long term goals.
  • Valid driver’s license. Ability to travel up to 75% including possible overnight or weekend stays.

Hiring Specifications Preferred:

  • Bachelor’s Degree.
  • Community health improvement program experience.
  • Non-profit or public economic development experience.
  • Program management experience.

Job Accountabilities:
a. Assist Iowa and South Dakota communities to identify built environment and policy opportunities that will transform the health of citizens. Engage community leaders and consult to aid the implementation of identified opportunities. Develop positive working relationships with community leaders local leadership, government, schools, and employers in order to influence the adoption of recommended environmental and policy changes.

b. Facilitate and manage a process to lead the community through an assessment process to identify environmental and policy opportunities which can improve wellbeing. While working lockstep with community resources, take a leadership role to analyze results of the assessment and direct development of a strategic Road Map comprised of programs, projects and initiatives to be implemented in the community. Influence a local volunteer leadership team to sponsor, execute and manage the Road Map.

c. Serve as a representative of Wellmark within the community. This may include speaking at local community events and organizations, participating in community events, facilitating community meetings and projecting a positive image of Wellmark and the partner organizations message. Influence work through volunteers within the community. Coach and plan activities and events with individual community leadership.

d. Facilitate the completion of reports and dashboards focused on process and outcome measures achieved within the community. This may include assessing citizen engagement, Road Map adoption and execution, etc. Provide periodic updates of key activities, achievements, and potential barriers to success to the local community leadership team and Wellmark leadership.

e. Engage Wellmark support resources as appropriate. This will primarily be members of the Sustainability Team but may also include Health Care Management Wellness Consultants work environment consulting, Network Relations Managers provider community, Sales client and potential client contact, Marketing Communications identify needed promotional and collateral materials public relations concerns, and Government Relations policy influence/collaboration.

f. Other duties as assigned.
