EXTERNAL SUMMARY

To provide accurate and timely assignment of appropriate diagnostic and procedural code(s) on the medical records for the purpose of collecting and indexing quality health information for routine patient types (outpatient diagnostic, outpatient physician practice/clinic, and/or emergency room encounters).

QUALIFICATIONS

• High School Diploma or equivalent, some college preferred.
• RHIT, CCA, CCS, CCS-P, CPC, and/or CPC-H required.
• Demonstrated knowledge of medical terminology, anatomy and physiology, pharmacology, and computers.
• One (1) year minimum experience in hospital and/or physician practice outpatient coding.
• Demonstrated problem solving and communication skills, demonstrated attention to detail, and demonstrated written communication skills that are legible to others.
• Demonstrated personal skills: organization and time management, human relations and teamwork, personal adaptability, personal motivation and listening

RESPONSIBILITIES

Reviews the medical record, super bill, and/or charge sheet to identify the diagnoses and procedures and assigns ICD-9&10-CM codes to routine patient types.

Identifies and assigns CPT-4 codes to all outpatient procedures.

Abstracts diagnostic and procedural codes and other pertinent data into the network system as defined in Policy and Procedures.

Reviews/monitors assigned work queues and missing documentation encounters as needed, and codes and abstracts any accounts that were missed.

Provides information on specific problem accounts to the Coding Supervisor.
