Case Manager, Mission United

United Way of Miami Dade is seeking for a Case Manager to join United Way’s Mission United Initiative. This position is responsible in providing case management along with service and resource coordination in a human services environment, particularly with veterans and military families as well as those with low income and homelessness related conditions. This initiative assist Miami-Dade military veterans as they re-acclimate to civilian life by providing them with case management and directed assistance with employment, education/training, legal and financial services, housing and healthcare access.

Principal Duties and Responsibilities:

  • Performs needs and comprehensive assessments; develops or updates care plans, service plan, and/or treatment plans with clients and their families to address their needs and to facilitate referral to appropriate treatment or services providers or facilities; engages clients and families in the development of service implementation plans
  • Counsels clients and/or sustaining success and wellness; conducts activities and monitors the service environment in a manner to maximize client success and well-being
  • Advocates on behalf of clients and families for services, basic needs, and other related issues
  • Composes or prepares correspondence, case notes, narrative and technical reports, notifications, and related documents using computer-based applications
  • Conduct comprehensive, client-centered social work activities in accordance with best practices
  • Conducts community and home visits (if needed) related to client needs; provides follow-up and monitors serve delivery and care environments
  • Stay abreast of community resources and procedures for referring clients to appropriate resources
  • Facilitates case consultation for client situations
  • Provides crisis prevention, intervention, and supportive counseling as needed.
  • Performs other duties and assumes other responsibilities as assigned by supervisor
  • Due to leadership role United Way is called upon to play in the community, particularly in times of crisis, it is the expectation that all United Way staff will be fully engaged in the organizations crisis plan and response efforts.

EDUCATION:

Bachelor’s degree required with major course work in social work, psychology or other related field. Military experience/Veteran highly preferred.

EXPERIENCE:

Minimum of two years’ experience in social case work, counseling, eligibility determination, social service or community resource referral, or closely related work. Case management background preferred. Knowledge of the needs of the Military/Veterans is essential.

SKILLS:

Possess excellent oral and written communication skills; strong public speaking skills; and ability to interact effectively with internal and external audiences/stakeholders, etc. PC literate with intermediate proficiency with Microsoft Office suite. Desired attributes include: initiative, self-direction, creativity, enthusiasm, team player, leadership skills, creative problem solving skills, good judgment, critical thinking and attention to detail.

Salary: $40,000.00 to $45,000.00 /year
