Salary Range : $36.46 per hour

Housing for Health (HFH) is a program office within Health Services Administration, a division under the Los Angeles County Department of Health Services (DHS). HFH was created and put into implementation in support of the Los Angeles County Homeless Initiative recommendations in response to and in support of the County’s effort to address and combat homelessness in the communities residing within Los Angeles County.

The DHS-HFH is establishing a Homeless Prevention Unit (HPU), which is a collaborative effort between the California Policy Lab (CPL), DHS-HFH, and the Department of Mental Health (DMH). The HPU will identify individuals who have used County systems of care who are most at risk of homelessness using predictive analytics and provide interventions that will address issues that might lead to homelessness. HFH in collaboration with DMH will be responsible for the overall planning, organizing, and directing the day-to-day operation of the HPU. It is expected that the HPU will have significant interactions with several County departments to address challenges navigating service systems and/or to connect or re-connect individuals to services to ensure housing stability therefore, the ideal candidate will be knowledgeable in mainstream and specific programs and services tailored to people experiencing homelessness while being flexible to meet the changing needs and demands of the program.

This position will serve as a program manager for the HPU and the program lead for DMH. Working closely with the DHS-HFH Senior Staff Analyst and DMH management, this position will assist with developing and implementing program structures, processes and evaluation measures and managing day-to-day operations for DMH.


ESSENTIAL FUNCTIONS
will include, but are not limited to, the following:

The HPU program manager will be responsible for the following activities:

  • Assist the DHS-HFH HPU Senior Staff Analyst with developing the operational structure of the HPU.
  • Assist the DHS-HFH HPU Senior Staff Analyst and DMH management with developing and implementing program processes for DMH, including day-to-day operations, reporting requirements and quality improvement/evaluation components.
  • Assist the DHS-HFH HPU Senior Staff Analyst and DMH management with identifying and developing processes for all potential interventions for DMH clients, such as access to sources of income, short term rental assistance, and assistance with arrears or utilities.
  • Assist with managing the DMH day-to-day operations of the HPU including reviewing client medical records and working with clinicians and case managers to determine and implement interventions and linkages to housing and services for referred individuals.
  • Assist with collaboration between the participating County departments and external agencies/community-based agencies.
  • Assist the DHS-HFH HPU Senior Staff Analyst and DMH management with coordinating and facilitating regular meetings and communications for HPU and program partners to ensure efficient and thorough processes to promote integrated, seamless and coordinated systems of care.
  • Assist the DHS-HFH HPU Senior Staff Analyst and DMH management with evaluating DMH program activities and progress, and help develop, recommend and implement program modifications.
  • Prepare program reports as needed including progress reports, performance data, and program outcomes.
  • Other duties as assigned.
  • JOB QUALIFICATIONS


    Minimum Qualifications

    Three years of experience in a staff capacity analyzing and making recommendations for the solution of problems of organization, program, procedure, budget or personnel -OR- Two years of highly responsible administrative or staff experience -OR- One year of responsible staff experience in health care or community or social service operations, -OR- Working in a program that provides housing and support services to clients who are homeless or are at risk for homelessness; -OR- Making recommendations for the design and/or implementation or evaluation of programs related to health and human services for high-risk populations (e.g. homeless, justice-involved, mentally ill, substance use disorder or medically complex)


    Desirable Qualifications

  • A Bachelor’s degree in Social Work, Psychology, Human Services or in a related field preferred.
  • Ability to pay close attention to detail.
  • Ability to work in a professional manner, with courtesy and tact, and maintain confidentiality and security of documents and information
  • Ability to work collaboratively with multidisciplinary professionals and partners both internally and externally
  • Must possess strong organizational skills initiative and follow-through.
  • Computer literate and skilled in use of MS Windows-based programs, including MS Office (Word, Excel, PowerPoint
  • Knowledgeable of data systems utilized across DHS and the homeless system of care, e.g., CHAMP, ORCHID, Clarity, Resource Management System, etc.


    Certificates/Licenses/Clearances

  • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
  • Successful clearing through the Live Scan process with the County of Los Angeles.
